    I don't see mentioned:

Advent: In Illo Tempore/To Dunsiname, and even better: Full Moon and Empty Hours/Riptide in Aeturnum
    Genesis: Cinema Show/Aisle of Plenty
Hatfield & The North: The Yes No Interlude/Fitter Stoke Has a Bath

I first heard the version on Live at Fairfield Hall and still prefer it. I think Mark Wedgewood's background vox make the difference for me. John G Perry is a great bass player, but his voice has a bit too much personality to do harmonies for my taste. (It reminds me of Terry Kath's harmonies on some Chicago stuff. A unique talent particularly ill-suited to composed close-harmonies.)

On the Chameleons "Script of the Bridge" the segue between the second song "Here Today" and the third song "Monkeyland" is awesome!

Nah, that was just a device for Steve and Mike to get more writing credits. But it does segue into 'Afterglow'.

Wigwam's Being contains nothing but twofer segue songs (although due to the limitations of vinyl format, one pair was split up in the middle).

Good choice, and I'd add Summer's Cauldron/Grass from the same album

Technically, this song doesn't belong as it's considered one song but it's really in the vein of Foreplay/Long Time and Funeral For A Friend/Love Lies Bleeding.

In the category of "segues that were composed and performed, but for some reason recorded as two separate pieces": Kansas' "Mysteries and Mayhem/The Pinnacle".

Prefab Sprout: “All Boys Believe Anything” → “The Ice Maiden” → “Paris Smith”

Yeah, desert island disc for me (provided of course that deserted island has electricity to keep my phone charged).
