A while ago I would have nominated the cover of "Strictly Inc." Strangely though I have come to appreciate it - I can't say I like it, but I find it funny. Perhaps I like the music so much that it's changed my feelings about the cover.
I have always thought this is in its own league, its the back side, but still
Muck Grohbian Muckefuck
There comes a point when a cover is so bad it's almost good - the Gong one for example.

This Steve Walsh solo album is quite dreadful.
Tony Banks "Strictly Inc."
Rush | first album: Actually very strong in terms of graphics, it's just those "we recorded all night at a studio at a discount rate and couldn't take a bath before this picture" pictures on the back cover that ruin it. Ironically, even though Gary Lee Weinrib has one of the worst photos on a debut album, he now seems to be the band member that women are nuts about. Go figure.
Yes | 90125: After years of wishing Yes would become more popular, they finally did, and I then didn't like their music. The cover really isn't that bad as a graphic cover idea, but as a band that had some of the best covers in progressive rock with Dean, it wasn't necessarily ugly. That would come with the next studio album - one of the worst covers ever, just from the design/color choices.

I blame Wynton, what was the question?
There are only 10 types of people in the World, those who understand binary and those that don't.
In what universe does Love Beach fit the description stipulated in the OP?
epic music, great musicianship, massively popular
And if there were a god, I think it very unlikely that he would have such an uneasy vanity as to be offended by those who doubt His existence - Russell

Pretty damned funny review of the cover art, regardless of the music.
You say Mega Ultra Deluxe Special Limited Edition Extended Autographed 5-LP, 3-CD, 4-DVD, 2-BlueRay, 4-Cassette, five 8-Track, MP4 Download plus Demos, Outtakes, Booklet, T-Shirt and Guitar Pick Gold-Leafed Box Set Version like it's a bad thing...
