Hard to go past Consequences IMO:

Perfectly captures the tone of the album.

Does it matter that this waste of time is what makes a life for you?

Rick Wakemans "No Earthly Connection" was cool, especially with the inclusion of tin foil.

KC - Lizard - the music goes perfectly with the cover. Its like another world.
Anthony Phillips - The Geese & The Ghost - also the music goes with the cover
Yes - Tales - love the pictures, that (while reading the lyrics) come up during instrumental passages.
rotters club also matches the music well, as does per un amico.

I know theres more. Flower Kings have some great packaging for instance.

For packaging, I thought King Crimson's Epitath was brilliant and compact considering how much it held.

Amon Düül II: Tanz der Lemminge
Banco del Mutuo Soccorso: Io sono nato libero (original fold-out LP artwork, which I sadly do not own)
Gentle Giant: In a Glass House
    Osanna: Palepoli
Premiata Forneria Marconi: Storia di un minuto
Todd Rundgren: A Wizard/A True Star
Semiramis: Dedicato a Frazz (what is it with Italian bands and these incredible cover artwork/packages?)

Marillion did a great job with Marbles, Happiness Is the Road, and Sounds that Can't Be Made.
